What we collect
Name, email address, WhatsApp number, whether you want to explore or offer services, language, permissions, and the date, policy version and source of registration. We do not request health data, identity documents or other people’s data.
Why we use it
To record and manage your expression of interest and contact you by email or WhatsApp about this request. Only if you select the optional permission will we also send launch news and promotional communications by email. That optional permission does not include WhatsApp marketing.
Where it is processed
The page is served through Cloudflare. Registrations are stored in Colombian Care Match’s Supabase project in the us-east-2 region (United States). These providers supply infrastructure services. We do not sell this form’s data or publish a list of registrations.
How long we keep it
We keep these registrations for up to 12 months from submission, unless you request deletion sooner. Expired records are scheduled for daily deletion. You can withdraw optional permission at any time without affecting the handling of your request. Aggregate counters do not contain form data.
Your rights and how to use them
You can request access, updates, corrections, deletion, withdrawal of permission or proof of the permission you gave. Contact the privacy address below, state the right you want to exercise and use your registered email or phone number to help us verify your identity. Do not send medical information or identity documents through this form. We will acknowledge your request and explain the next steps. You may lodge a complaint with Colombia’s Superintendencia de Industria y Comercio after completing the applicable process with the controller.
Measurement without form content
We use Cloudflare operational metrics and aggregate counters for page requests, submission attempts and saved registrations. We do not send names, emails, phone numbers, form fields or registration identifiers to analytics tools. We do not use personalized advertising, session recordings or tracking cookies. After saving a registration, a temporary confirmation cookie is used to show the thank-you page; it expires after 10 minutes.
